New Patients
If you would like to have an appointment with us we would be happy to see you. As Dr Yeoh is a board registered specialist, you will need a referral from your dental or medical practitioner. A referral form can be found on our site that can be given to your medical or dental practitioner to complete.
We will endeavour to see you as soon as possible if your referring practitioner considers your problem an urgent one and requests an urgent appointment for you.
Medical History and New Patient information
We ask all new patients to complete a questionnaire that details your contact information as well as your previous and existing medical history. If you would like to complete this form at home and bring it with you to your appointment click on the below link.

Fees And Rebates
Dr Yeoh is a dental specialist and therefore fees are not rebatable from medicare and are only rebatable from private health insurance with the appropriate dental cover. Please check with your health fund to see if you are eligible to claim a rebate.
Please note that payment for all services is required on the day of treatment. Payments can by made by cash, eftpos or credit card (Visa, Mastercard, American Express)
